printed on the back.
This format, of course, only allows one person to
answer the survey.
Households with more than one member
(‘couples’ or the new ’family’ offer) can still
proceed here to register the levels of interest in different areas of
activity at BLT of a 2nd, 3rd, 4th person etc.
Please tell us where your main levels of interest
lie at BLT by ticking the boxes.
NOTE: It doesn’t matter which method you
use - ‘renewal form’ or ‘online’ - but only one
survey can be accepted from each member.
